At Gold Coast Home Health Inc., we provide wound care, diabetic care, cardiopulmonary support and care, infusion therapy, orthopedic services, and many other medical conditions. Here are a few of the many benefits you and your loved ones can get from our home health care services:
Lesser travel from home to the hospital. Treatment is directly administered in the client’s home in collaboration with your primary care physician and others.
A specialized care plan will be designed for each client, taking into consideration the client’s individualized needs. One-on-one care ensures that the care services are properly administered under the supervision of your physician, a registered nurse from our agency, or therapists from our contracted agencies.
Services are available 24 hours a day, 7 days a week, with schedules that can be adjusted per the client’s healthcare requirements and or special needs.
Direct coordination with your physician on your response to your individualized care plan. Adjustments to such a care plan can be easily made upon request or as necessitated by your health conditions.
We utilize the services of specialists such as Dr. Ofisi for our individualized and highly successful wound care program. We utilize the services of seasoned, well-tested consultants who work with accreditation bodies for Medicare. One of the consultants we use is 5 Star Consulting. They provide continued staff training, in-house surveys, and chart reviews to improve care, patient outcomes, and competency. Our care is evidence-based practice that empowers our clients’ rehabilitation and speeds recovery.
The following steps are reviewed to prevent rehospitalizations and maintain the agency’s goal.
- Was the assessment on the start of care complete?
- Were appropriate disciplines ordered based on OASIS?
- Were frequency and duration appropriate?
- Were visits front-loaded?
- Was MD notified of any changes in the patient’s condition?
- After a change in condition, was the visit frequency increased if necessary?
- Did disciplines communicate with each other regarding patient change?
- If the patient/caregiver called the RN after hours, did the on-call RN make a visit?
- If the patient was non-compliant with orders, was the MD called?
- Was appropriate patient/caregiver teaching done?
- Was the response to teaching documented?
- Was a pain assessment done at every visit?
- Was there anything the agency could have done to prevent the hospitalization?
